Six cases of insecticide poisoning involved ingestion of fenthion with suicidal intent were described. The progression of symptoms in the patients followed previously described patterns including acute cholinergic crises and the intermediate syndrome. Evidence of extrapyramidal involvement was noted between 4 and 40 days after poisoning. Extrapyramidal symptoms included dystonia, choreo/athetoid limb and trunk movements, tremors, dysarthria, and cog wheel rigidity. The extrapyramidal features lasted between 1 and 4 weeks and spontaneously resolved in surviving patients.
